This agreement releases Coach 46 Goalkeepers Club and the City of Oceanside from all liability relating to injuries that may occur during goalkeeper trainings and goalkeeper clinics. By signing this agreement, I agree to hold Coach 46 Goalkeepers Club and the City of Oceanside entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
I also acknowledge the risks involved in goalkeeper trainings and goalkeeper clinics. These include but are not limited to breaks, sprain, strains, bruises, dislocations, or concussions. I swear that my goalkeeper is participating voluntarily, and that all risks have been made clear to me. Additionally, my goalkeeper does not have any conditions that will increase their likelihood of experiencing injuries while engaging in goalkeeper trainings and goalkeeper clinics.
By signing below, I forfeit all right to bring a suit against Coach 46 Goalkeepers Club and the City of Oceanside for any reason. In return, my goalkeeper will receive participation in goalkeeper trainings and goalkeeper clinics. I will also make every effort to ensure my goalkeeper obeys safety precautions as listed in writing and as explained to them verbally. I will ask for clarification when needed.
